Some groups treated the chatbot as a search engine, receiving a list of possible diagnoses without an explanation of the reasoning behind them. Other groups proposed hypotheses and asked GPT-4 to assess them, receiving aligned results. Rheumatoid arthritis was suggested as a likely diagnosis, despite being relatively low on the group’s list of possibilities. However, Gout was considered improbable due to the patient’s demographics.
To extract the maximum benefit from GPT-4, instructors emphasized the importance of framing the interaction correctly. Providing the chatbot with a patient description and listing symptoms prior to seeking a diagnosis mimics the process of consulting a medical colleague, enabling students to delve deeper into the bot’s reasoning. It is crucial to recognize that chatbots can make mistakes or produce results lacking factual basis, so using them accurately and with caution is paramount.
Dr. Byron Crowe, an internal medicine physician involved in the project, drew a parallel to the aviation industry, stating that pilots rely on GPS for navigation, but airlines maintain high standards for reliability. Similarly, in the field of medicine, using chatbots as thought partners is acceptable, as long as the expertise of human medical professionals is not replaced. The chatbot’s value lies in augmenting doctors’ abilities, not replacing them.
In the case of the patient with the swollen knee, the true diagnosis turned out to be Lyme disease, a possibility that had been proposed by every group and by GPT-4. While the chatbot successfully agreed with the groups’ conjectures, it offered no additional insights or an illness script.
